Hacked twice by the same person on different platforms

In the last few months I have had someone trying to change my passwords and 2 factor. First on my ps4, which i stopped. However they succeeded in hijacking my instagram, the weird part is both times they used a phone number ending in the same last 4 digits. Nobody I know would be able to do this or any enemies I can think of.
